Indiana Primary Office Information

Indiana office located in a small town, right on the main street is leased from Floyd Memorial Hospital. The office is located directly across the street from the hospital in the professional arts building. The office is on the ground floor.  The lease expires December 2025.

Kentucky Satellite Office Information

Dr. Chatham owns his satellite office condo. Approximately 1400 square feet. Location is East-end Louisville condo office is where many business offices are, and there is a Baptist-owned hospital. It’s a “Mid to upscale community.”
